COMMUNITY HEALTHCARE EAST IS SEEKING EXPRESSIONS OF INTEREST FOR
COMMUNITY CONNECTOR SERVICE
The Head of Service for Health & Wellbeing in CHO6 is inviting
expressions of interest from organisations to deliver a Community
Connector service, aligned to the social prescribing service (by
employing and managing a community connector).
Successful organisations will be contracted by HSE via Section **Apply
on the website** Grant Agreement (Health Act **Apply on the website**)
to deliver a specified level of service.
Previous experience in delivering a social prescribing project is not
essential
